By the way, this document can be included along with the application of your I-485 adjustment of status. It is free if you are applying for it along with your I-485 adjustment of status. If you came to the United States as a K1 visa holder, you can use your married last name as your last name in the form. For the middle name, you can use your mother's maiden name. Leave the province section blank. For country, put USA. For other information about me, I don't have an alien registration number yet, so I will leave that blank.
