If you came to the United States as a K1 visa holder, you can use your married last name as your last name in the form. For the middle name, you can use your mother's maiden name. In the physical address section, since I just arrived in the United States, I don't have an official mailing address yet, so I'm going to put my husband's name and our address: (address details). Leave the province section blank. For country, put USA. For other information about me, I don't have an alien registration number yet, so I will leave that blank. For country of birth, put Philippines, and for country of citizenship, put Filipino. For class of admission, put K1 fiance. For gender, put female, and for date of birth, put your date of birth. Proceed to the next page and check the first option that says "I am a permanent resident or conditional resident of the United States" since you are applying for a re-entry permit.
